The directors and staff of the Sam L. Cohen Foundation maintain a continued interest in the organizations and projects we support. Your report enables us to review our grant-making programs and evaluate more effectively the impact of our grants. In most cases, final grant reports are due 13 months after the award date.
Please submit your report using the Foundation’s online system. To access your account, select the Account Login button below and enter your login information. The Grant Report Form is available as a link under “Requirements.” Similar to the application, you may save and review your work prior to submitting it.
A Financial Statement is also required. For project grants, provide a project budget with actual costs and all income sources. For operating support grants, provide the organization’s budget versus actual report for the fiscal year in which the grant was expended.
